Microdissection Testicular Sperm Extraction for Non Obstructive Azoospermia | TESA | PESA
What is Micro Tese/M-Tese
Microsurgical testicular sperm extraction (microTESE) is a surgical procedure used to retrieve sperm from the seminiferous tubules of a male’s testes. It’s used for men with non-obstructive azoospermia, which is when a man cannot produce enough sperm to have a detectable amount in his semen.
Where is it indicated?
Operative sperm retrieval is indicated in men with obstructive azoospermia (OA) if reconstruction is not possible, or has failed. In men with OA, sperm may be retrieved percutaneously by percutaneous epididymal aspiration (PESA) or by open surgical procedures, e.g. microsurgical epididymal sperm aspiration (MESA) or the simpler procedure of open fine-needle aspiration (OFNA) that does not need an operating microscope or by a single open testicular biopsy. In any case, sperm retrieval is easy.
How successful is Micro Tese?
Successful sperm retrieval has been reported in up to 63% of men undergoing micro-TESE The greater challenge is sperm retrieval in men with non-OA (NOA): only some of these men will have a few sperm in the testes, and the distribution of these scanty sperm may be multi-focal or very localized, necessitating different sperm-retrieval techniques.
How long does a TESE procedure take?
The simplest and most cost-effective is an aspiration of sperm. This is routinely performed under local anesthesia and takes approximately 10 minutes. Harvesting sperm from a man with a testicular problem using microscope is much more difficult and often takes several hours.
Is Tesa procedure painful?
With the TESA method, patients often have pain and some swelling after the procedure. PESA and TESA methods are also a treatment option for men who are experiencing non-obstructive and obstructive oligozoospermia.
